Suspends payments and freezes your account for up to 3 months if you are unable to work due to an approved leave of absence: you must (1) be off work for at least 30 days in a row, (2) on an employer approved unpaid leave of absence to attend to the needs of an Immediate Family Member due to an accident / illness, or to attend to the needs of a newborn or newly adopted child, and (3) from a job you worked an average of at least 30 hours each week for 30 days prior to the date of Qualifying Event. |

Unemployment
Suspends payments and freezes your account for up to 9 months if you involuntarily lose your job. You must experience an involuntary loss of employment that lasts at least 30 consecutive days and worked an average of at least 30 hours each week for the 30 days immediately preceding the date of the Qualifying Event. |

Temporary Disability
Suspends payments and freezes your account for up to 18 months if you are unable to work due to temporary disability. You must (1) be off work due to an injury or illness for at least 30 days in a row, from a job worked an average of 30 hr. / week over the 30 days prior to the date of Event, and (2) be under a licensed physician's care for the duration of the injury or illness. |

Permanent Disability Cancellation
Cancels your account balance if you are unable to work due to a permanent disability. You must (1) be injured or ill, that prohibits ever performing any work for income, and (2) have worked an average of at least 30 hours each week over the 30 days ending on the date of the Event. |

Long-term Temporary Disability Cancellation
Cancels your account balance after temporary disability suspension benefit has been activated for more than 18 months. You must have been approved for the Temporary Disability benefit, and have had the Temporary Disability Debt Suspension benefit activated for the 18 months immediately preceding the Cancellation. |

Long-term Unemployment Cancellation
Cancels your account balance after the Unemployment Suspension Benefit has been activated for more than 9 months, immediately preceding the Cancellation. |
